Terms & Conditions for Internship Consent Form

1. Eligibility & Applicability
   This consent form is valid and mandatory only for 5th semester students of Purnia University enrolling in the internship program conducted by Krayog Enterprises. It is not applicable to any other program or institution.
2. No Payment Clause
   This form does not require or request any payment, OTP, or financial information during or after submission. Students are advised not to share such details under any circumstances.
3. Final Submission Policy
   Once the form is submitted, no changes or edits can be made. Students must ensure that all information provided is accurate and complete before submission.
4. Pre-Registration Requirement
   Students must have successfully registered for the internship program and completed the required payment prior to filling out this form. Submission without prior registration may lead to rejection.
5. Invoice Verification
   Students must have their invoice number ready at the time of filling out the form. The invoice can be downloaded from the Krayog application. This is required for verification purposes.
6. Document Requirements (Prerequisites)
   Before filling out this form, students must ensure they have the following documents ready for upload:
   • Passport-size photograph
   • Signature
   • 4th Semester Admit Card
   • 5th Semester Admission Slip
7. Authenticity of Information
   All information and documents submitted must be genuine and accurate. Any false or misleading information may result in disqualification from the internship program.
8. Data Usage Consent
   By submitting this form, the student agrees that their provided information may be used by Krayog Enterprises for academic, administrative, and internship-related purposes.
